Registering with the surgery
To register with the surgery:
You don’t need proof of address or immigration status, ID or an NHS number to fill out the form.
You will be registered within 48 hours.
This service is part of the NHS, so your personal information will be kept safe.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Please Note: If you are a Hartpury College Student, please ensure that when you complete the New Patient Registration form that you enter Hartpury College as your home address.
